Scheduling an appointment
Prior to scheduling an appointment, call your insurance to see if Dr. Hawkins is an approved provider and to receive pre-authorization. If Dr. Hawkins is not on your insurance and you still would like to be seen please contact our office to make an appointment. You will be considered a cash pay patient seen fee-for-service. In the event your insurance provider changes or you acquire mental health coverage, it is your responsibity to notify our staff as soon as possible. You will need to present your new insurance information at your next appointment. It is imperative that you provide this info to our staff before you see Dr. Hawkins. This will give our staff opportunity to verify benefits and notify you of any changes in copays or deductibles. Any fees associated with services provided not covered due to failure to notify us of any policy changes will be your financial responsibity. All fees are due at the time service is rendered. Individuals without valid photo identification(i.e. driver’s license, ID card, military ID) will not be seen. Also a valid social security number for the insured will be required for biling purposes. Please bring your pre-authorization information on the day of the appointment.

Dr. Hawkins treats adolescents ages 16 and older and adults, including the geriatric population.
Cancelling an appointment
For new patients, in the event you cannot make your appointment, it is asked that you send us a courtesy email at doctor.hawk@hotmail.com before end of business day on the day before your appointment. This will allow us to utilize that time for someone who may be in dire need of seeing the psychiatrist. Once you are an established patient, it is imperative that you cancel 24 hours in advance. This email will serve as written verification of the time and date you cancelled. Patients with no access to email must call 24 hours in advance to cancel. Missed appointments not cancelled 24 hours in advance will incur a $45 no-show fee.
